In-home Assessment

Our second engagement involves an assessment where we go to your home to answer your questions and we ask some of ours. This helps us understand your needs and level of care we need to provide and a care plan is developed.

Following the in-home assessment from where the care plan is developed, we match your needs with a caregiver with the skills and understanding to serve your unique needs.

We stay in touch with you and regularly support the caregiver to ensrue that you are receiving sufficient care. If there's a need for additional services or to modify your service or any other needs that arise, we are right by your side

FREE IN-HOME ASSESSMENT

We always insist on conducting an in-home assessment with our patients and their families, before agreeing to the terms of services and costs involved. These home visits are conducted free of cost and have the objective of understanding the patient’s situation and building, together, a care plan that makes sense for everyone.

A comprehensive home assessment will also help us to understand the home conditions and verify modifications that might be necessary to increase the mobility of the patient and help the caregivers to perform their duties.

It is our philosophy to treat home care in a holistic, comprehensive way, considering the environmental conditions of the home the level of care needed, to the personality and skills of the caregivers which will make them perfect for each patient.
 

During these assessments, we can determine if a patient needs Personal Care, Companion Care, or both and how much of each. We also help the families to plan for the future, as they also usually use this moment to reflect on their realities, responsibilities, ad stressful situations. And most important: whatever they decide, we are here to help!

UNDERSTANDING PENNSYLVANIA MEDICAID WAIVER

HELPING PENNSYLVANIANS LIVE INDEPENDENTLY AT HOME

The Medicaid Waiver programs present a new angle to traditional Medicaid, offering direct funding to Pennsylvania. These programs, notably, waive the requirement for the funds to be used strictly for medical purposes. Instead, they are structured to enable Pennsylvania residents to enjoy a life of dignity and independence in their own homes. By diverting individuals from costly institutional facilities, the Medicaid Waiver programs result in substantial Medicaid savings.

ADMINISTERED BY PENNSYLVANIA'S OFFICE OF LONG-TERM LIVING (OLTL)

These vital programs fall under Pennsylvania's Home and Community-Based Services. They are managed by the Office of Long-Term Living (OLTL), a part of Pennsylvania's Department of Public Welfare (PA DPW). Eligibility for these programs is determined based on an evaluation of your income and assets. To discuss your potential eligibility, you can reach out to the OLTL at 1-866-286-3636.

IF YOU HAVE NEVER RECEIVED A MEDICAID WAIVER SERVICES BEFORE, HERE'S YOUR GUIDE

The Medicaid Waiver journey starts by contacting the PA Independent Enrollment Broker (IEB) at 1-877-550-4227. The IEB is a private entity, contracted by the OLTL, providing expert assistance in guiding you through the program. An IEB representative will schedule a home visit within seven days of your request to explain the program and assist you with the paperwork. The representative will then coordinate the necessary steps, providing you written updates throughout the process.

The evaluation period can extend up to 90 days. On acceptance into one of the PA Medicaid Waiver programs, you will choose a Service Coordinator (SC) who will help streamline your personal care.

Provider (Amaka Passion Homecare Services): Delivers home care.

Amaka Passion Homecare proudly serves the Aging Waiver, Attendant Care/Act 150, COMMCARE, Independence Waiver, and OBRA waiver programs. Our focus is on providing identical, high-quality care to all clients, whether they are Medicaid waiver, insurance, or private pay clients. We offer a free assessment with a registered nurse (RN) or licensed nurse practitioner (LNP), supervised continued care, 24-hour phone access, and all of our web-based access tools.

We respect your right to choose or change your care provider. You can do so without impacting your program eligibility. If you wish to transition to our care, call us directly at 610.880.8988. We can arrange the details and ensure a smooth transition within a few days.

KNOW MORE AOBUT THE WAIVER PROGRAMS

Here's a brief overview of the Waiver programs that Amaka Passion Homecare services. Visit the PA State website for more details on each program. All of these are crafted as alternatives to assisted living facilities or other health care institutions.

Aging Waiver: For Pennsylvania seniors over 60 needing skilled nursing care at home.
Attendant Care / Act 150 Waiver: For physically disabled adults (18-59) capable of managing their own care and affairs.
COMMCARE Waiver: For adults over 21 suffering a traumatic brain injury that requires home care.
Independence Waiver: For severely physically disabled adults (18-60) requiring nursing facility care.
OBRA Waiver: For adults (18-59) with developmental physical disabilities like cerebral palsy, epilepsy, etc.

2. IN-HOME CARE ASSESSMENT: We will answer all your questions, and ask some of our own to craft a Customized Care Plan for your unique needs.

3. START OF CARE: We will come with our Caregiver on the first day of care to go through the Care Plan together and help with introductions.

4. CLIENT VISITS: Based on your needs and preferences, we visit regularly or place you on our 24/7 check-in program
