ACT 150 provides support and services so people with physical disabilities can continue to live at home. The state-funded services are offered for a fee to persons who do not qualify financially for Medicaid. Eligibility is based on a level of care assessment, a financial eligibility determination, and program eligibility.
To Be Eligible for Act 150 Program Services, You Must:
Be a resident of Pennsylvania.
Meet the level of care needs of a Skilled Nursing Facility.
Be between 18-59 years of age.
Be capable of - Hiring, firing, and supervising attendant care worker(s) - Managing your own financial affairs - Managing your own legal affairs
Be capable of directing your own care.
Have a medically determinable physical impairment that is expected to last for a continuous period of not less than 12 calendar months or that may result in death.
Proceed through the eligibility process for the Medicaid (MA) program and be determined financially ineligible.
Require assistance because of physical impairment to complete daily living functions, self-care, and mobility, including but not limited to, those functions in the definition of Personal Assistance Services.
May have a low co-payment based on your income. The co-payment will not be more than the total cost of services.
